Akron Children's is the largest pediatric provider in Northeastern Ohio and one of the fastest-growing children's hospitals in the U.S.
CHALLENGE
Healthcare providers are transforming their delivery models from fee-for-service to value-based care to improve their overall operational performance and quality of patient care. Successfully adopting a PCMH model of care was the foundation for Akron Children’s Hospital’s transition to value-based care, but the organization had to overcome significant technological, operational, and cultural gaps to achieve its objectives.
SOLUTION
CTG worked with Akron Children’s to transform across four areas: operations; IT and reporting; population health management; and quality. On the IT side, many important Epic developments were needed, and CTG also helped develop analytics dashboards to monitor progress and performance in real-time. Care coordination services were implemented to help improve transitions of care and encourage greater engagement between patients and their parents.
